Support at Home

Support at Home Program provides coordinated services to help clients remain safe, independent and connected while continuing to live in their own home and community. Services are tailored to each person’s assessed needs and may include home and community supports, advisory and support services, nursing care, personal care, care support and care management.

The pricing schedule is based on Aboriginal Community Services’ Support at Home Program rates, effective 1 November 2025.

Rates vary by service type, unit, time of delivery and applicable loading.

This incorporates:

  • Standard and non-standard hour service pricing
  • Saturday, Sunday and public holiday pricing
  • 30-minute, hourly, trip, meal and 15-minute billing units where applicable
  • A 10% administration fee for brokered or variable services not listed
  • Specific administration fees for assistive technologies and home modifications

Transport trips are priced based on a standard distance of up to 30 kilometres per trip.

Support at Home Funding Classifications

Each participant receives one of eight ongoing Support at Home funding classifications based on assessed care needs. The quarterly budgets and annual amounts below are effective from 1 November 2025 and include 10% allocated for care management.

Administration fees: Services not listed, including brokered or variable services, include a 10% administration fee on the invoiced amount. Assistive technologies attract an administration cost of 10% of the time cost, or $500, whichever is lower. Home modifications attract an administration cost of 15% of the time cost, or $1,500, whichever is lower.
